Reducing Chronic Disease Disparities through Electronic Health Record/Health Information Technology (EHR/HIT) and Team-Based Care (TBC) Grant

Wisconsin DHS CDPP is seeking up to three health care clinics to partner under the Reducing Chronic Disease Disparities through EHR/HIT and TBC Grant. Each eligible clinic may apply for up to $20,000 for a quality improvement project. Projects will use standardized CQMs to identify priority populations, and then implement new or enhanced multi-disciplinary TBC approaches to achieve maximum impact and reach for priority populations. Projects will also incorporate rapid, continuous quality improvement processes throughout the award period.
