Minority Health Mini Grants: Community Grants

The Division of Public Health (DPH), Wisconsin Minority Health Program (MHP), under authority of the Wisconsin Department of Health Services (DHS), will award mini grants pursuant to Wisconsin Statute §250.20 for the purpose of conducting activities to improve the health status of economically disadvantaged minority group members. It is the intention of the Department to award multiple (approximately 10-12) contracts, ranging from $12,500 to $50,000 each, for the Community Grants for the services required in this solicitation. Funding for this grant announcement is contingent upon approval of the appropriation by the Wisconsin Legislature. The Program reserves the right to use this request for applications to award any additional funding that may become available. All applications and additional documents must be received via email no later than Wednesday, May 27, 2020, at 08:00 a.m. CST. Email applications and additional documents as WORD or PDF document attachments to: DHSWIMinorityHealthProgram@wisconsin.gov All questions about the RFA and the process need to be submitted by April 29, 2020. Late, hard copy or faxed proposals will not be considered. The State of Wisconsin reserves the right to reject any and all applications.
